How are we doing?  In our effort to provide quality services to our community, we have developed a survey regarding services you received from the Peoria Police Department Victim Assistance Program.  This survey is anonymous in order to obtain honest and candid feedback from victims for continual program improvement. There are a total of 14 questions, please select the answer which is most appropriate for each response and feel free to add any comments.
