Open Enrollment for the 2019 - 2020 Plan Year has ended as of May 16, 2019. If you need to make a mid-year change to your benefits as the result of a Qualifying Event, please contact Human Resources for assistance with updating your Benefit Election Form.

Benefit elections made during the annual Open Enrollment are effective July 1, 2019 - June 30, 2020.

 

Beneficiary Designation Form
IMPORTANT: Human Resources has implemented the Beneficiary Designation Form for employees to elect who they want to have their final paycheck issued to in the event they pass away during the course of employment. HR advises all employees to complete this form and submit to HR to keep on file. This is a form similar to other beneficiary forms that can and should be updated as needed. This form can be found in the menu to the right titled "City of Peoria Beneficiary Designation Form."
