Arizona Summer Classic Championships - REGISTRATION

Registration Fees & Insurance Requirements:  
  $800 per team if registered before June 1, 2011
  $850 per team if registered after June 1, 2011

Minimum deposit of $200 is due at registration; deposit is refundable up to 15 calendar days prior to the start of tournament. Balance of registration is due 15 calendar days prior to start of tournament.

Proof of insurance required in amounts of $2 million general aggregate and $1 million per occurrence. City of Peoria must be listed as additional insured. Proof of insurance must be submitted 15 days prior to start of tournament..

To register for the Summer Classic Championships, take the following steps:

          Step 1:   Register to play in the Summer Championships.

                         Option 3:  Fax the official Entry Form to (623) 773-8716.


         Step 2:  Make payment.
                       
                         Option 1:  Mail a check, payable to City of Peoria, to the address above.
                                      
                         Option 2:  Call (623) 773-8700 to pay with a credit card.
