PEORIA AM/PM RECREATION PROGRAM Sponsored by: City of Peoria & Peoria Unified School District August 13, 2007 - May 22, 2008 PROGRAM INFORMATION WHAT The Peoria AM/PM Recreation Program provides an opportunity for children to participate in a variety of activities before and after school. The goal of the program is the total well-being of each and every child. The Program provides a daily core schedule and an instructor visits various sites throughout the year, giving participants the opportunity to explore new experiences that will expand learning. Classes may include ballooning, structured sports, drumming, sign language, and tutors in a supervised, safe and secure environment. WHO Students in grades K-6. Children in all day Kindergarten may attend both AM and PM care. AM Kindergarten may attend AM care, PM Kindergarten may attend PM care TAX ID #- 86-6003634 Please save your receipts for tax records SUMMER YOUTH PROGRAMS Summer Camp Who: Entering K - 6 Location: Various Elementary Schools in the City of Peoria Dates: May 23– August/ date TBA Time: 6:30am – 6:00pm, Monday-Friday Cost: $23 per day or $97 per week Talk about fun in the sun!! This camp is all that and more! Summer camp offers all-day childcare with a daily field trip, special lunches and, of course, a number of “BIG” field trips for example; Big Surf, Barnum & Bailey Circus and the Mercury Game. Combining all of this with onsite activities is awesome summer plans at Peoria Summer Camp. There is a one-time registration fee of $30. $20 of the registration fee will be waived for 2006-2007 AM/PM participants, and 2007 returning Summer Camp participants who provide full payment with registration on or before August 3, 2007. After August 3, 2007, full fees will be required. All participants must make full payment to attend the program. The daily fee includes the field trip, such as swimming, movie, or library. A Summer Camp t-shirt is included in the registration fee. DES funding is available for qualified applicants. Summer Recreation Who: Grades 1-5 Locations: Various Elementary schools in the City of Peoria Dates: May 27-July 17, 2008 Monday – Friday, 8:00am – 2:30pm Cost: $50/residents, $90/non- residents for 8 weeks (t-shirt included). This exciting program features on-site activities and off-site field trips in a part-time, drop-in format. Children may choose to sign themselves in/out at any time during the program hours. If you would like licensed daycare supervision or extended hours, please refer to the Peoria Summer Camp Program. Daily off-site field trips are offered for an additional Fee. Summer Recreation t-shirts are included in the registration fee. Extra Summer Recreation T-shirts are available for purchase for $10.00 or 2 for $15.00. They will be exposed to a variety of activities that promote opportunities for choice, experimentation, and enhance self-esteem. Dates: August 13, 2007 – May 22, 2008 Time: 9:00am – 11:30am, Monday-Friday Cost: $9 per day or $45 per week Peoria Lil’ Learners Too is a sister program to the Lil’ Learners all day program. This is also a state licensed program and reflects all the ideas and goals of the full-time program with part-time hours. Peoria Lil’ Learners Too offers flexible scheduling. Tiny Tots Summer Recreation Program Locations: Parkridge Elementary, Paseo Verde Elementary and Peoria Elementary Dates: May 27-July 17, 2008 Monday – Friday, 8:00am – 2:00pm Cost: $50/residents, $90/non- residents for 8 weeks (t-shirt included). INSURANCE: The City of Peoria carries liability insurance for all its operations, including City-sponsored recreation programs. However, the City does not carry medical insurance for any participant's injuries or illnesses associated with any specific recreational activity. Medical claims are your responsibility. PERSONAL ITEMS – The program provides a variety of toys and many activities to keep your child busy and entertained. The City of Peoria is not responsible for personal items brought to site. Children are not allowed to use cell phones at site. If you and your child need to be in contact for urgent matters, we will allow occasional use of site phone for short calls, 2-3 minutes in length. HOLIDAYS and BREAK CARE All program sites will be closed on the following holidays: September 3 - Labor Day November 12 - Veteran’s Day November 22 & 23 - Thanksgiving January 21 - MLK Day February 18 - Presidents’ Day All day “Break Care” is offered on these days. Columbus Day - October 8 Winter Break - December 21, 24, 26-28, 31, January 2-4 Spring Break - March 24-28 Testing Break - April 25 Early Release & Parent/Teacher Conferences EARLY RELEASE DAYS AM/PM Recreation Program will provide childcare on the scheduled professional development days at time of release at 11:00/11:40 pm. Days will vary for each school. Care will NOT be provided for Kindergartners until school’s early release time. Participants enrolled 5 days a week in the PM program consistently will not be charged an additional fee for early release days. All 0-4 day schedules will pay a $14 fee for the PM program that day. Lunch will not be provided on these days. Children will be required to bring a non-perishable sack lunch. PARENT TEACHER CONFERENCE DAYS October 18 & 19 February 14 & 15 Children will be released at 10:00/10:40am from school. AM/PM will be available at that time. Participants enrolled 5 days a week in the PM program will not be charged an additional fee. All 0-4 day schedules will pay a $14 fee for the afternoon. Children will be required to bring a non-perishable sack lunch. No transportation or field trips will be provided. SCHEDULE, FEE & PAYMENT POLICIES SCHEDULE CHANGES Any deviation from your submitted schedule on the enrollment form must be given to the site the Thursday prior to the week the changes are to take effect. Participants may not show up and attend the program without prior notification to the program. If your child needs to attend on short notice, parents must call the AM/PM site to make arrangements. Payment is due the day of participation or before. A $5.00 per day/ per child late fee will be charged until payment is made. Children will not be accepted into the program without prior notice ABSENCES If your child will not be attending the Peoria AM/PM Recreation Program as scheduled, please call the AM/PM Recreation site prior to school dismissal. We have 24 hour voicemail available; please leave a message. Credit will not be issued for days missed. DELINQUINT ACCOUNTS/LATE PAYMENT FEE Payment is due by 6:00pm the Thursday before the week of participation. A LATE FEE OF $5 PER DAY/PER CHILD will be assessed for each day payment is late. If payment, including late fee, is not paid in full by Wednesday of the following week, the participant will not be allowed to attend until the account is cleared. RETURNED CHECK FEE (RCF) A returned check will result in service/program interruption. RCF of $30.00 per check and the amount of the returned check must be paid in cash, cashier’s check, credit card or money order at Community Services Department in City Hall or over the phone with a credit card by calling 623-773-7137. All future payments will be required in cash, cashiers check, or money order for a period of six (6) months from the date of the returned check. AM/PM FEE ASSISTANCE The Department of Economic Security (DES) has funding available for low-income families who would like to utilize the program. Contact DES at (623) 846-1046 to find out more information on how to apply. Processing can take up to 30 days. SPECIAL ACCOMMODATIONS Parents of a child with a disability may request a reasonable accommodation by completing a “REQUEST FOR ACCOMMODATION” form and submitting supporting documentation at least 2-3 weeks prior to date attending. Life Skills such as, but not limited too, feeding, dressing and toileting are not provided by staff. For more info you may contact Community Services at 623-773-7137. POLICIES and PROCEDURES SIGNING IN & OUT A child enrolled in the Peoria AM/PM Recreation Program will only be released to persons specifically authorized with their signature on the registration form. NO exceptions will be made without the advance written permission or telephone authorization from the parent or guardian. Individuals will be required to show a valid picture I.D. at the time of pick-up. If only one person has the sole legal custody of a child, then legal custody papers must be on file at the AM/PM site. A.M. - Participants must be signed in daily by a parent or authorized person. This is done by signing your full name and time on the form. Children may not sign themselves in. The AM/PM site staff is authorized to sign the child out and release the child to the school. P.M. - For the safety of your children, we require that a parent or authorized person sign the children out each day by writing their full name and time on the form. Children may not sign themselves out. The AM/PM site staff is authorized to sign the child into the PM program. LATE PICK UP FEE A $15 fee per child will be assessed for every 15 minutes past 6:00pm. 6:01pm-6:15pm:$15; 6:16pm-6:30pm:$30, etc. Fee assessment below. Pick-up time will be recorded by the designated site clock. 1ST - incident will result in FEE plus verbal warning 2ND - incident will result in FEE plus written warning 3RD - incident will result in FEE plus a three-day suspension 4TH - incident will result in FEE plus a one-week suspension 5TH - incident will result in FEE plus removal from the program HEALTH and EMERGENCY PROCEDURES If your child becomes ill during the program, we will place your child in an isolated area. According to the time of day, and the degree of illness, the Site Director will decide to call the parents and make arrangements to have the child taken home by an authorized person (not a staff member) or keep them at the AM/PM Site. Please don’t send your child if they missed school or displays any of the following symptoms; nausea, rashes, red/inflamed eyes, temperature above 99° without medication, or cold symptoms within the past 24 hours. Minor scratches and cuts will be treated at the site. If a serious accident should occur, the City of Peoria Fire Department will be summoned for treatment and they will decide whether your child should be taken to the nearest hospital or doctor's office by ambulance. You will be notified immediately. Treatment may only be rendered if we have your written authorization on file. POLICIES and PROCEDURES DISCIPLINE The staff will implement an assertive discipline program. Children involved in our program are expected to follow the rules and directions of the AM/PM staff. The following guidelines are used when disciplinary action becomes necessary due to unacceptable behavior. Parents will be notified if their child needed guidance or discipline. The course of action below will be followed in normal circumstances. Extreme behaviors will accelerate the process and be dealt with on a case by case basis: Extreme behaviors are, but not limited to: Physical attacks such as, hitting, biting, spitting, kicking, etc., abusive language, disrespect to staff (verbally or physically by child or parent), leaving the program area without permission, repeated offenses within the same day or those behaviors that put a child or others in danger. (No refund for suspensions or early pick-ups on scheduled days) PARENT/STAFF COMMUNICATIONS The AM/PM Recreation Program staff would like to work as a team with school and family to provide the best environment for the child's growth and development. Exchanges of information between parents and staff will be a formal or informal format. Information that is shared regarding issues in school, AM/PM or any changes occurring within the family for example, a sibling moving, a sick grandparent or pet, or alterations in the parents’ relationship will give all of us insight to a child’s behaviors or attitudes. MEDICATION The Peoria AM/PM Recreation Program staff may administer medication. To authorize giving medication to a child, parent/guardian must complete a “Medication Release Form” available at the site and bring the prescribed amount of medication in the original container. Apply sunscreen as applicable prior to attending Break Care days. 1. Warning - for specific unacceptable behavior 2. Time-out - with a warning of future consequences for repeated behavior. 3. Time-out - with a warning and/or write up for repeated behavior in file and parent notified upon pick-up. 4. Time-out - with a call to parent or guardian and a write-up like above. Discuss corrective action & consequences for future incidents with parent upon pick-up. 5. Suspension - 1 - 2 scheduled days from the program and/or the remainder of the day. *Repeated aggressive/inappropriate behavior with 3-5 suspensions will result in removal from AM/PM with approval Supervisor.
