Library Cards are free to
anyone who lives in Maricopa
County.

Eligibility
Obtaining a library card is
required to utilize library
services. All patrons wishing to
obtain a Peoria Public Library
card must provide identification
(picture identification and
proof of current address). It is
necessary to have current
address and telephone number on
file for each library cardholder
so we may contact them when
necessary. This also ensures
patron confidentiality and
expedites service.
Types of identification that are
acceptable:
Drivers license or picture ID
and any one of the following:
personal checks, postdated
current mail or voter
registration card, etc.
Patrons who cannot provide two
forms of identification (one
photo and one with current
address) at the time of
application will not be able to
receive a library card.
Any
child under the age of 18 MUST
have a parent or guardian
present to obtain a card. A
parent or guardian must provide
appropriate ID. There is no age
restriction on who may receive a
library card. Parents are
responsible to monitor library
materials their children utilize
and check out.
The
patron must be present when the
card is issued. Replacement of
lost library card is $2.00.
Cards may be used at the Main
Library or the Sunrise Mountain
Library.
Library Card Application
Things to remember:
If
you allow someone to use your
library card, YOU are
responsible for any fines that
accrue for misuse of your card.
That includes fees for replacing
lost materials as well as
overdue fines.
When
you move, don't forget to
include the library on your list
of places to notify of your new
address.
Lost
Library Cards:
Report the loss or theft of your
library card immediately. The
last thing you want to do is pay
for materials someone else has
charged to your card. You can
call the Main Library
