Spring Break Camp
March 24-28

The Recreation Division will again offer a program to accommodate parents requests for child care during Spring Break.  The program will feature arts and crafts, games, sports and the opportunity to attend the Spring Break Trips.

Remember - your child does NOT need to be registered in the Spring Break Camp to attend our trips!  Just sign up on-line and meet the bus at the designated school.

PROGRAM INFORMATION:

Participants:  Children enrolled in K-6 grades.

Hours:  6:30 a.m. - 6:00 p.m. 

Fees:  $23 per day includes the Spring Break trip for that day. A $5 registration fee per child will be charged for any child not currently enrolled in the Peoria A.M./P.M. Recreation Program.  Checks made payable to "City of Peoria Day Camp."  
ALL FEES DUE AT TIME OF REGISTRATION.

Lunch:
Parents are required to supply a sack lunch for their child(ren) each day.  The program will provide a morning and afternoon snack for participants.  Optional special lunches will be offered on various days for $3.00.  Morning and afternoon snacks are provided.

Registration:  Those not currently enrolled in the 2007-2008 AM/PM Program must complete a Spring Break Day Camp registration packet, including:  Immunization records, four emergency contacts (name, address & phone number), and doctor/hospital information

SPRING BREAK TRIPS
If your child is enrolled in Spring Break Camp, they are automatically pre-registered for the SPRING BREAK TRIPS.  No additional trip fees or registration forms are needed.  Parents are only required to sign an authorization form when dropping off their child each day.  Children enrolled in Day Camp will be picked-up and returned to their Day Camp Site.

PROGRAM ATTENDANCE GUIDELINES

Registration is ongoing through March 28th, and WILL BE TAKEN AT AM/PM SITES or can be mailed /delivered to the Community Services Department at Peoria City Hall @ 8401 W. Monroe St., Rm. 180, Peoria, AZ 85345.

Full payment must accompany the Fee Attendance Contract at the time of enrollment, including the Spring Break Camp registration fee (if applicable and is NON-REFUNDABLE)

Notification of schedule changes or enrollment status must be made by Monday, March 24th. Notification must be received in writing, or verbal verification can be made by calling Cree, Jenny or LeAnne @ 623-773-7139 or 623-773-7192 or 623-773-7505.

24 hr. voicemail is available.

Flexible schedules are accommodated, however days must be chosen in advance and paid prior to attending.

No refunds will be issued after March 24, 2007. REFUNDS OR CREDITS WILL NOT BE GIVEN FOR ILLNESS OR MISSED DAYS.

Any additional attendance fees, late fees, etc., are due that day, upon the time of pick-up .

Failure to pay any balance due may result in the child being dropped from the program.

Late Fee: A late fee will be charged at a cost of $15 per 15 minute intervals per child.

(6:01 - 6:15 = $15, 6:16 – 6:30 = $30, and so on….)

An $30 RCF fee applies to all returned checks.

DISCIPLINE: Children enrolled in the program are expected to follow all school & program guidelines, as well as staff direction, at all times. This is for the safety of all program participants. Should discipline become necessary the

following steps are implemented:

Verbal warning for specific unacceptable behavior

Time-out: 1 to 5 minutes

Time-out: 5 to 10 minutes

Written warning signed by parent and child

Temporary suspension or removal from the program

Extreme behaviors (physical attacks, abusive language, disrespect to staff, leaving the program area without permission, etc.), or those that put a child or others in danger will accelerate this process.
