WINTER BREAK CAMP

 January 2-4, 2008

The Peoria Recreation Division, in partnership with the Peoria Unified School District and Deer Valley School District, offers a recreation program to accommodate your need for licensed childcare during Winter Recess. The program provides a variety of activities, including arts & crafts, group games, sports, guest instructors and daily off-site trips.

This program is available to currently registered AM/PM participants as well as new patrons.

PROGRAM INFORMATION:

Safety:

A parent, or authorized person, must sign a child in/out of the program each time they attend.

Highlights:
Winter Break Camp  features licensed staff, nutritious snacks, and a well-balanced program.   The program provides an opportunity for children to participate in a variety of recreational activities and develop social skills in a supervised and safe environment.  Children will also enjoy daily field trips.

Parents must send a non-perishable sack lunch and drink each day.  Morning and afternoon snacks are provided.  There will be optional special lunches offered on various days for $3.

Registration:  
Registration packets are available at Peoria City Hall, Room 180, at the Peoria Community Center, the AM/PM sites during program hours or at any elementary school office within Peoria.  Children not already enrolled in the 2007-08 AM/PM program must complete a Winter Break Camp registration packet, including immunization records and information for four (4) emergency contact, to enroll their child in the program.  Completed registration packets may be returned to City Hall or to an AM/PM program site prior to December 21.  Mailed registration will be accepted before December 20.

For those participants currently enrolled in AM/PM, a Fee Attendance Contract must be submitted with your payment.

PROGRAM FEES
A $5 NON-REFUNDABLE registration fee and all camp fees are due at the time of registration for all new participants.  Existing AM/PM participants will pay only the program fees at the time of registration.  All fees include daily field trip and morning & afternoon snack.

*** PROGRAM ATTENDANCE GUIDELINES****

Welcome to the Peoria Winter Break Camp Program.  Addressed below are several of program policies in order to avoid misunderstanding.  Thanks again for trusting us with your child care and recreation needs.

Registration is ongoing through January 4 at City Hall and Camp sites. AMPM Sites will accept registrations through December 20 before the end of the AM program. Registrations can also be mailed or delivered to the Community Services Department at Peoria City Hall at 8401 W. Monroe, Rm 180, Peoria, AZ 85345 until 12/18.  Registrations will be accepted at the new Community Services Offices located at 9875 N. 85th Ave. from 12/19 to 1/4/2008.

Full payment must accompany the Fee Attendance Contract at the time of enrollment, including the Winter Break Camp registration Fee, if applicable and is NON-REFUNDABLE.

Notification of schedule changes or enrollment status must be made by December 20th.  Notification must be received in writing, or verbal verification can be made by calling Cree or Jenny @ 623-773-7139 or 623-773-7192 (24 hr. voice mail available).

Flexible schedules are accommodated, however days must be chosen in advance and paid prior to attending.

No refunds will be issued after December 22rd.  REFUNDS OR CREDITS WILL NOT BE GIVEN FOR ILLNESS OR MISSED DAYS.

Any additional attendance fees, late fees, etc. are due at the time of pick-up that day.  Failure to pay any balance due may result in the child being dropped from the program.

Late Pickup Fee:  A late fee will be charged at a cost of $15 per 15 minute interval.  (6:01 - 6:15 = $15, 6:16 - 6:30 = $30, and so on...)

A $30 Returned Check fee applies to all returned checks.

DISCIPLINE:  Children enrolled in the program are expected to follow all school and program guidelines, as well as staff direction, at all times.  This is for the safety of all program participants.  Should discipline become necessary the following steps are implemented:

  1.  Verbal warning for specific unacceptable behavior
  2.  Time-out:  1 to 5 minutes
  3.  Time-out:  5 to 10 minutes
  4.  Written warning signed by parent and child
  5.  Temporary suspension or removal from the program

For extreme behaviors: (physical attacks, abusive language, disrespect to staff, leaving the program area without permission, etc.), or those that put a child or others in danger will accelerate this process.

 

Make checks payable to "City of Peoria".   The City cannot accept post-dated checks.
