Spring Break Camp
March 18-22
The Recreation Division will again offer a program to accommodate parents requests for child care during Spring Break. The program will feature arts and crafts, games, sports and the opportunity to attend the Spring Break Trips.
Remember - your child does NOT need to be registered in the Spring Break Camp to attend our trips! Just sign up on-line to participate in our Spring Break Adventure Trips program and meet the bus at the designated school.

Fees
$24 per day includes the Spring Break trip for that day. A $5 registration fee per child will be charged for any child not currently enrolled in the Peoria A.M./P.M. Recreation Program. For new registrations, please attach a copy of your child’s immunization records to the completed Emergency Information form with no blanks and must have exact addresses. Your child will not be able to attend without the completed Emergency form or a copy of immunization records.
Checks made payable to "City of Peoria Day Camp." All fees are due at the time of registration.
Lunch
Parents are required to supply a sack lunch for their child(ren) each day. The program will provide a morning and afternoon snack for participants. Optional special lunches will be offered on various days for $4.00.
Spring Break Camp Registration
All participants must complete a fee attendance contract and permisson form.
Those not currently enrolled in the 2012-2013 AM/PM Program must complete a Spring Break Day Camp registration packet, including: Immunization records, four emergency contacts (name, address & phone number), and doctor/hospital information. Your child will not be able to attend without the completed Emergency form or a copy of immunization records.

Spring Break Trips
If your child is enrolled in Spring Break Camp, they are automatically pre-registered for the Spring Break Trips. No additional trip fees are needed. Parents are only required to sign an authorization form that is found in the registration packet. Children enrolled in Spring Break Camp will be picked-up by a school bus (not airconditioned) and returned to their camp site.
The Spring Break Adventure Trips program is offered to children who only want to participate in the trips and not the camp. Children are transported to trips by school bus (not airconditioned) and supervised by City of Peoria Recreation staff. All trips will leave and return from Cheyenne, Coyote, and Paseo Verde. Please register for only one of the locations. No food or drink can be brought on the bus ride. We recommend that sun screen be applied prior to attending any outdoor trips. Program Attendance Guidelines
Registration is ongoing through March 22rd, and will be taken at AM/PM sites, or can be mailed /delivered to the Community Services Department at 9875 N. 85th Avenue, Peoria, AZ 85345.
Full payment must accompany the Fee Attendance Contract at the time of enrollment, including the Spring Break Camp registration fee (if applicable and is non-refundable)
Notification of schedule changes or enrollment status must be made by Monday, March 18th. Notification must be received in writing, or verbal verification can be made by calling Cree or LeAnne @ 623-773-7139 or 623-773-7505.
24 hr. voicemail is available.
Flexible schedules are accommodated, however days must be chosen in advance and paid prior to attending.
REFUNDS OR CREDITS WILL NOT BE GIVEN FOR ILLNESS OR MISSED DAYS.
Any additional attendance fees, late fees, etc., are due that day, upon the time of pick-up .
Failure to pay any balance due may result in the child being dropped from the program.
Late Fee: A late fee will be charged at a cost of $15 per 15 minute intervals per child.
(6:01 - 6:15 = $15, 6:16 – 6:30 = $30, and so on….)
An $30 RCF fee applies to all returned checks.
Discipline:
Children enrolled in the program are expected to follow all school & program guidelines, as well as staff direction, at all times. This is for the safety of all program participants. Should discipline become necessary the
following steps are implemented:
Verbal warning for specific unacceptable behavior
Time-out: 1 to 5 minutes
Time-out: 5 to 10 minutes
Written warning signed by parent and child
Temporary suspension or removal from the program
Extreme behaviors (physical attacks, abusive language, disrespect to staff, leaving the program area without permission, etc.), or those that put a child or others in danger will accelerate this process.
