A scheduled meeting of the Public Safety Retirement Board, representing 
Fire employees of the City of Peoria, Arizona was convened at 8401 W. 
Monroe Street in open and public session at 8:30 a.m. 

Members Present:  	Mayor Bob Barrett , Chairman
			Mark Barbee, Member
			David Block, Member			 
			Barry Goodman, Member
			Anthony Neely, Member


Staff Present:  	Nancy Fantasia, Human Resources Analyst
			Dee Campbell, Executive Assistant


Others Present:  	Steve K. Decker 

	
Call to the Public:	There being no public present, Chairman Keegan called 
for the first agenda item.
        
        
Audience:  3 people.

Note:  The order in which items appear in the minutes is not necessarily the 
order in which they were discussed at the meeting.
        
        
Minutes:

Barry Goodman moved to approve the minutes of May 15, 2006, as presented.  Motion 
was seconded by Mark Barbee and carried unanimously.


NEW BUSINESS:

Application for Accidental Disability Retirement:	 

Nancy Fantasia advised the Board Steve K. Decker submitted an accidental disability 
retirement application.  

Mark Barbee moved to accept the application of Steve K. Decker for accidental 
disability retirement.  Motion seconded by Anthony Neely and carried unanimously. 

The Board was then advised that an Independent Medical Examiner (IME) should be 
appointed  pursuant to ARS Section 38-847.D.9 to perform an independent medical 
evaluation of the applicant. The Board nominated Dr. Gerald Yacobucci, 2222 E. 
Highland, Ste #300, Phoenix, AZ 85004 to be the IME, with Dr Stuart Kozinn, 7301 
E. 2nd St, Ste 102, Scottsdale, AZ 85251 as a back-up choice as IME in the event 
Dr. Yacobucci was unavailable.

Barry Goodman moved to accept Dr. Gerald Yacobucci as the primary IME with Dr Stuart 
Kozinn as a back-up choice for accidental disability retirement.  Motion seconded by 
David Block and carried unanimously. 

Adjournment:

Being no further business to come before the Board, the meeting was duly recessed 
at 8:42 a.m.
